Satisfactory Academic Progress (SAP) Appeal Form - Students are required to meet the standards of SAP as defined in the MGC catalog. If a student is suspended from financial aid eligibility due to his/her failure to maintain SAP, he/she may complete the SAP Appeal Form.

Dependency Status Appeal Form - A student's response to the questions in section 2 of the FAFSA determine whether or not a student will be classified as dependent or independent. It is possible to appeal a student's dependent classification under certain documented circumstances.

Budget Revision Request Form - Federal Student Aid regulations permit the Financial Aid Office to consider budget revisions for the purchase of a computer, child care and/or housing costs.

PLUS Denial Notification Option Form - If your parent applied for a Parent PLUS loan and received notification of a denial as a result of the credit check, you must complete the PLUS Denial Notification Option Form. This form instructs the FAO regarding your interest in an additional Unsubsidized Stafford Loan.

Transient Students - Students who wish to study as a transient student will need to complete the appropriate agreement.
Consortium Agreement - This form must be filled out by students who have been granted permission by the MGC Registrar's Office to take classes at another institution as a transient student. This form must be sent to the host school's Financial Aid Office and returned for processing. All transient students must be prepared to pay all fees and tuition at the host institution. MGC will disburse aid on the published disbursement date.

Hope Transient Agreement - This form must be filled out by students who have been granted permission by the Registrar's Office to take classes at another institution and are eligible to receive the HOPE Scholarship. Upon receipt, MGC will notify your host school regarding your HOPE eligibility. The host school will disburse your HOPE award.

Federal Work Study Application - Students must have a 2.0 GPA and enrolled in at least 6 hours to be eligible for the Work Study program. Please complete the following application and return to Mrs. Deborah Teston in the Financial Aid Office. Eligible students will be awarded on a first come first serve basis as funding for the program is limited.
