APPLICATION PROCESS

STEP ONE: FILL OUT THE ONLINE APPLICATION |

STEP TWO: AGREE TO THE TERMS OF THE ONLINE ORIENTATION |
This is included at the end of the application and is required for membership. |
STEP THREE: SIGN REQUIRED FORMS |
Download, print, sign and return the (Appendix B - Assumption of Risk.PDF) form.
Download, print, sign and return the (Appendix C - Waiver of Liability...) form.
NOTE: You are not required to download these forms. They will be available at orientation.

STEP FIVE: ID CARD. |
Obtain ID card from the campus card office located in the "Community Center/Police Dept." beside the lake.(You will be required to show your ID card each time you visit the Wellness Center.)

STEP SIX: PRESENT CARD AT EACH VISIT |
Present your MGC ID Card at the front desk of the Wellness center for access to the facility.
The Wellness center will keep a list of members who have completed these steps.The Business Office will notify members of card expiration.
